Why did so many die in the tornadoes?

First of all, I am very saddened by the devastation in the South and plan on donating. But why did so many people die? Don't they have tornado warnings and shelters? Kindly enlighten me. I don't mean this in a bad way at all. I just don't know much about living in an area prone to tornadoes.
